New Patient Forms
Records Release Form
We are striving to provide quality time for your appointment. We will not double book patients or squeeze your important visit into a tiny time slot! In order to optimize your time with the doctor, it is most important that you arrive on-time for your scheduled appointment. As a result, our policy is that will reschedule you if you are more than 15 minutes late. Also, if you cannot make your scheduled appointment, please inform us at least 24 business hours in advance.
It is your responsibility to notify us if you cannot keep your appointment. If you do not you’re your scheduled clinic or ultrasound appointment or arrive more than 10 minutes late, you will be charged a $50 fee. If you do not keep your saline infusion sonogram (SIS) appointment, you will be charged a $75 fee.
If you are a new patient and have not completed the new patient forms, please come 20-30 minutes prior to your scheduled appointment to allow time to complete them in the office.
