Records Release Form
We are striving to provide quality time for your appointment. We will not double book patients or squeeze your important visit into a tiny time slot! In order to optimize your time with the doctor, it is most important that you arrive on-time for your scheduled appointment. As a result, our policy is that will reschedule you if you are more than 15 minutes late. Also, if you cannot make your scheduled appointment, please inform us at least 24 business hours in advance.
If you do not show for a scheduled appointment, or show up more than 15 minutes late, you will be subject to a $30 fee.
If you are a new patient and have not printed and completed your patient packet., please arrive 20-30 minutes before your scheduled appointment to complete in the office. Otherwise, we may have to reschedule your visit.
